Abstract:
Der Pflegeentlassungsbericht - auch Situationsbericht genannt - bildet zusammen mit dem Arztbrief das sogenannte Entlassungsdokument. Beide, sowohl der Pflegeentlassungsbrief als auch der Situationsbericht sind wichtig für die Weiterbehandlung und Betreuung des Patienten.<br />In Österreich gibt es jedoch keine einheitliche Form dieses Pflegeentlassungsberichts. Jedes Bundesland und im Weiteren auch jede Krankenanstalt verwendet unterschiedliche Formen und Systeme. Infolge dessen gibt es auch keine einheitliche Terminologie, das heißt, die Bezeichnungen für einzelne Bereiche des Pflegeentlassungsbriefes sind von Land zu Land unterschiedlich.<br />Aber nicht nur in medizinischen Einrichtungen, sondern auch in Organisationen, wie zum Beispiel dem Fond soziales Wien, Caritas, Hilfswerk, usw., unterscheiden sich die einzelnen Berichte voneinander.<br />Aus diesem Grund befasst sich diese Diplomarbeit mit dem Versuch, eine einheitliche Form für den Pflegeentlassungsbrief zu entwickeln. In Zusammenarbeit mit der Medizinischen Universität Wien und dem Wiener Krankenanstaltenverbund (KAV) wird zudem noch eine einheitliche Norm für Österreich angestrebt. Die bereits vorhandenen Berichte sollen dabei als Hilfestellung und Anregungen dienen. Des Weiteren ist es wichtig, sich auch an internationalen Standards und Systemen zu orientieren. Unter Berücksichtigung und Miteinbeziehung der Vor- und Nachteile lassen sich Fehler bei der Entwicklung vermeiden, um ausgehend davon einen optimalen Standard entwickeln zu können.<br />

If a patient leaves the hospital, he gets a discharge document, which is important for the further treatment and care. This discharge document includes the physician's letter as well as the care discharge report.<br />Although Austria is a small country, there is no standardized form of the care discharge report existing yet. Each state, even each hospital uses different forms and systems. In fact, there does not exist a consistent terminology in Austria. In Vienna, for example, the care discharge report is called situation report. Also the designations of the individual domains have not been standardized until now.<br />Not only medical constitutions but also other organisations like "Caritas", "Fond of social Vienna" or "Aid Organisation" (Hilfswerk) use care discharge reports.<br />To maintain the patient's optimal treatment and care after discharge, the development of a standardized form of the discharge care report is indispensable.<br />For that reason, this diploma project occupies with an attempt to develop a standardized form for the care discharge report. In association with the medical university of Vienna and the Vienna hospital group a standardized norm for Austria will also be aspired. The already existing reports should thereby be an assistance and suggestion. Additionally it is also important to orient oneself by international standards and systems. In consideration and incorporation of the advantages and disadvantages, mistakes and errors can be avoided, in order to develop an optimal standard.
